Liability Waiver

I hereby affirm that I am voluntarily starting a course of instruction in physical fitness and performance training (the “Activity”) with Karyn Ravin, NASM Certified Personal Trainer (the “Trainer”) of Karyn Ravin Fitness. I am voluntarily participating in the Activity entirely at my own risk at a space provided by me or at a space provided by the Trainer (the “Premises”).

Assumption of Risk: I understand and agree that if I engage in any physical exercise or activity recommended by the Trainer, including personal training, or enter the Premises or use any facility or equipment on the Premises (or at my own residence or facility) for any purpose, I do so at my own risk and assume the risk of any and all injury and/or damage I may suffer, whether while engaging in physical exercise or not.  This includes injury or damages sustained while and/or resulting from using any premises or facility (including the Premises), following any regimen or protocol recommended by the Trainer, or using any equipment (mechanical or otherwise), whether provided to me by the Trainer or otherwise, including injuries or damages arising out of the negligence of Trainer, whether active or passive, or any of Trainer’s affiliates, employees, agents, representatives, successors, and assigns.  I further agree that I am voluntarily participating in personal training and related activities and assume all risk of injury, illness, damage, or loss that might result whether arising out of the negligence of the Trainer or otherwise.

Release: In full consideration of the risk of injury while participating in the Activity, and for the right to participate in the Activity and enter the Premises, I, on my own behalf and for myself, my heirs, executors, administrators, assigns, or personal representatives, hereby knowingly and voluntarily waive and release any and all rights, claims or causes of action of any kind whatsoever arising out of my participation in the Activity and/or my utilization of the Premises or equipment located therein and further release and waive any claims that I might have against the Trainer, or her affiliates, managers, members, agents, attorneys, staff, volunteers, relatives, heirs, representatives, predecessors, successors and assigns, for any kind of risks related to traveling to and from as well as participating in the Activity, which may include, but are not limited to, physical or psychological injury, pain, suffering, illness, disfigurement, temporary or permanent disability, economic or emotional loss, and death.

I acknowledge that I have carefully read this form and fully understand that it is a release of liability. I expressly agree to release and discharge Trainer, or her affiliates, managers, members, agents, attorneys, staff, volunteers, relatives, heirs, representatives, predecessors, successors and assigns, from any and all claims or causes of action and I agree to voluntarily give up or waive any right that I may otherwise have to bring a legal action for personal injury or property damage.
